Brotherhood Community Care

About the service
Brotherhood Community Care Southern and Brotherhood Community Care Northern provide care packages to people needing assistance to live at home, and those who care for them.
The services we offer range from assistance with daily needs, such as personal care, shopping and meals, through to home nursing and specialist health services.
Our care packages are designed to meet individual needs. Each client is assigned a dedicated care manager who works with them, and others involved in their care, to plan and coordinate their package. Packages are constantly reviewed to ensure clients are receiving the best level of care possible.
Who is eligible?
A person may be eligible for Brotherhood Community Care if:
- they need support to remain in their own home OR their carer needs respite
- they are unable to arrange care services for themselves
- they live in the cities of Port Phillip, Stonnington, Bayside, Casey, Cardinia, Greater Dandenong, Frankston, Glen Eira or Kingston, or in the Mornington Peninsula Shire (Brotherhood Community Care Southern)
- they live in the cities of Yarra, Moreland, Hume, Darebin, Whittlesea, Banyule or Nillumbik (Brotherhood Community Care Northern)
- they have been assessed by an approved assessing agency, such as ACAS (Aged Care Assessment Service).

Brotherhood Community Care offers:
- individually tailored care packages
- a dedicated care manager
- ongoing care planning and care management
- help with practical needs such as personal care, continence management, housekeeping, meals and nutrition, home maintenance, gardening, shopping and transport
- support with home nursing and specialist health services, such podiatry and physiotherapy
- recreational and community activities
- assistance with obtaining mobility equipment.
